Hallucination
Definition. A hallucination is a confident, fluent statement produced by a language model that is factually wrong: an invented statistic, a misattributed quote, a citation to a study that does not exist. It is the single largest editorial risk in AI-assisted content.
Hallucinations are most dangerous where they are most plausible: precise-looking numbers, specific dates, named sources. Those are exactly the details readers and editors are least likely to challenge.
The mitigation is procedural, not technological. Every number, date, price and quotation in AI-assisted content needs verification against a named source before publication.
Retrieval-augmented systems reduce the rate but do not eliminate it, because a model can still misread or overstate what it retrieved.
